Maximilian
The built-in Surface Blur tends to solve this sort of problem with relative degrees of success depending on the effect's settings and picture quality, though sometimes the result looks rather unnatural and therefore unsatisfactory. If the noise is too evident, I'm afraid it would be better to consider the possibility of taking a new picture with a better quality (remember that the quality of the camera you use has a lot to do with this). You can also try Smart Blur, which is part of this pack.
AndrewDavid
The image you posted was in the JPG format. I copied and saved as a PNG and applied the built-in Reduce Noise effect at default settings and came up with this improvement, See what you think At this point I do not see any room for improvement. But i am not a professional

I tryed the blur in paint.net but not smart blur.
Maximilian
Click on the thumbnail below for the full-size version. It's the improvement I could make by combining Surface Blur, Gaussian Blur, TR's Dodge and Burn (in blur mode), and a few subtle touches with the Smudge tool. I hope it helps.
