Currently you have to select inside the dropdown to change the brush size via mouse scroll wheel. Outside of the dropdown it seems that [ and ] and Ctrl + [ or ] can change the brush size too. Keyboard & Mouse Commands (getpaint.net)

 

With the default left hand position in the WASD area it could be more convenient if say the unused combos Ctrl + Shift + Scroll wheel or Alt + Scroll wheel could change the brush size as well.
