If you are also using the latest version of pyrochild plugins, then I don't know.
Maybe run PdnRepair.exe

You can make a circular outline using the Ellipse tool with Shift key down.
(Not Ellipse Select tool.)
No adjusting nubs, but you can make a Rectangle selection around the circle that you have made,
and adjust the size using the Move Selected Pixels tool with Shift key down.

Draw your rectangle on a new layer. (Transparent)
Use one of the Outline Object plugins to add an outline to its border.

Make a selection with the Magic Wand or other selection tool, then press Delete.
Save as PNG or GIF.
That's one way to make transparency.
There are a number of ways to create transparency.
The most appropriate way will depend on the particular image that you are working on.

Try this:
Install Simon Brown's Printer+ plugin.

Open your resized image.
Expand the canvas size to full A4 size, anchored Middle.
Image > Canvas Size...
Print using the plugin, set to A4 with zero margin size.
Your image should print out at the size that you want it to be in the center of the page.

When you have jagginess in a small area, like the teeth and blood trail here,
it is feasible to do anti-aliasing as pixel art/pixel editing.
Zoom in and color-pick a color adjacent to where you'll be editing.
On a new layer, add a few pixels to fill in, then reduce the layer opacity.
Make another new layer and repeat until it looks right.
Here's a PDN file with the edits to the tooth on the viewer's left.
Switch the visibility of the top layer on and off to see the difference.
You can press F4 on various layers to get a sense of the layer opacities that are typical.

You can save the layers in PDN or 32-bit PNG format for later use.
Both formats are non-lossy.

Paint.NET doesn't include any fonts.
It uses the fonts in your Windows/Fonts folder.

Also, from the appearance of the icons in your screenshot,
your computer's file association for .dll is now Paint.NET.
That is not normal.
If you want to restore the .dll file association to normal default,
you might want to make use of this tutorial:

OK. Those aren't brushes, those are effect plugins.
And they do belong in the Effects folder, not the Custom Brushes folder.
If you have them installed in the Effects folder already, close and open Paint.NET.
If they still don't show in the Effects menu or a sub-menu,
make a full-size screenshot of your open Effects folder
showing the contents and the full path in the address field.

Create your numbered circles on a new (transparent) layer.
Save that layer as a PNG file set to read-only.
Any time you want to use it, import it into your new project:
Layers > Import From File
Make a Rectangle select around the number that you need, leaving transparency all around the object.
Then, copy and paste into a new layer above your working background image.
Non-finicky selection method.

You can use the Liquify plugin, set to Pucker, Pressure 1%.
Make several clicks and/or sweeps in the central region of the text.
