    Perhaps you could use the "Median plugin".
    Steps:
    1. Duplicate your image and run the Median effect on the upper layer.
    2. Erase with a soft brush the areas that you want to keep sharp, like the eyes, eye brows, nostrils, lips and teeth.

    I have been exploring some of the plugins and I think you may achieve this with a Polar Transformation distortion.
