Yes, you can figure out what the shellex is and remove it. Shell extensions are things that show thumbnails for files Windows wouldn't normally, or add things to the right-click menu. Paint.NET uses one to show thumbnails for .PDN files.

I believe DivX is a known culprit, so if you have that, try removing it. You could also try disabling thumbnails completely by going to Performance Settings. Unfortunately, I don't remember how to get there on Vista.

Another tool to help nail that down is to look at the list of DLLs in the crash log above. This will include DLLs for shell extensions, although I'm not seeing anything suspicious in your list. Anything in c:\Windows is probably fine, as well as C:\Program Files\Paint.NET obviously.

 

More importantly it'll tell you which shell extension DLLs were actually loaded into Paint.NET's process, as opposed to showing you all that are registered. This narrows down what could be causing it. In this case I'm not seeing any (not even Paint.NET's own ShellExtension_[x86/x64].dll), so if it's due to a shell extension then it's a problem with the DLL being loaded. I doubt that though.
