so how could i fill it in with white and keep the colored border about 1.5 in thick

i have a picture of a ocean wave and i want to rep[lace the water with the word change repeated several times ( to represent waves of change) how would i do this? any help is welcome

im looking for something ti this affect im trying to get the heart in the rib cage but i have two seperate images and when i copy and past one ever the ribs get blocked rather then the heart being blocked by ribs. any help is appreciated.
